Currently when you request access there is a simple text box.

Would be helpful if there was an ability to a specific message. Add some simple fields for them to fill in. (I accept this is a slippery slope and suddenly you have to reimplement all of User App workflows here) But some basic ability to request info (mandatory or optional) no lookups.
