When a Reviewer selects Remove, add a pop-up window that asks if the user is sure that Removal is their choice. Additionally, we want to be able to customize the text in the window so we can refer the Reviewer to site-specific staff for any questions.

We think that this would preclude accidental Removals, or Removals in which the Reviewer is not familiar with the user. Also, we want text that can be customized so that we can enter site-specific information that is relevant to the Reviewer.
