I would take this a step further and say any "orphaned" items should escalate in a similar manner.
Another take from a new prospect: If a reviewer was a manager of N employees with M reviewable entitlements each, and as a result of the reviewer's termination, each of the N direct reports got reorganized to N new managers, then all N*M review items should be "smart" enough to redirect themselves to the N new managers' review inboxes.
